The three ways UK locums get paid

1. Flat day rate. Common for NHS-heavy practices and short-notice cover. You agree a fixed figure for the day regardless of what you produce. Simplest to negotiate; caps your upside.

2. Percentage of gross. Typical in private and mixed practices — you take a percentage of the fees you generate. This rewards a busy day but exposes you to a quiet book or a practice that under-fills your list.

3. Per-UDA (NHS). You are paid per Unit of Dental Activity delivered. Your effective day rate depends entirely on the UDA value and how the list is booked.

What drives your number

  • Region. London and the South East command more than the North East or Wales, but cost of living and competition follow.
  • Notice. Same-day emergency cover pays a premium. Regular booked days pay less.
  • NHS vs private. Private percentage days can out-earn flat NHS days on a busy book — and under-earn on a quiet one.
  • Your evidence. A locum who arrives with a portfolio and clean records is worth more to a practice than one the principal cannot vet.

How to negotiate without underselling

Do not open with "what do you pay?" Open with your rate and a reason for it. "My day rate is X. Here is my case portfolio and my indemnity is in place for the full scope you need."
